Using the Accu-Chek® + 360Care™ app conveniently through a mobile device, you’ll have access to a team of Certified Health Practitioners dedicated to supporting you through a personalized health plan.
Getting started is easy. You’ll receive an Accu-Chek® Guide blood glucose monitor delivered right to your home along with a 3-month supply of testing strips. The Accu-Chek® Guide meter conveniently connects to the 360Care app allowing you to easily monitor your blood sugar levels. Additional supplies will arrive automatically as you need them.
With Accu-Chek® + 360Care™ you can hold live video sessions with your team of health coaches (nurses, dietitians, diabetes educators, psychologists/social workers), each dedicated to supporting you through a personalized health plan to improved health outcomes, and happier living.

The Concierge’s role is to provide an introduction to the program, assist with the Accu-Chek® + 360Care™ app download and installation, schedule your first meeting with your Certified Health Practitioner (health coach) and answer any questions that you might have about the program.

A healthcare practitioner with a specialization in diabetes education, will discuss your health goals, current condition, lifestyle choices, and barriers with you, and then develop a tailored course of treatment that meets you at your current level of health, ability, and engagement.
Focusing on small incremental changes, your health coach will guide you through attainable milestones, and schedule coaching sessions for you around diet, nutrition and mental health and wellness delivered directly through the app.
The Accu-Chek® + 360Care™ Program brings together key clinical data from your smartphone and integrated wireless health measurement devices, giving you and your coaches a full view of up-to-the-minute health and progress. The app and smart device integrations provide an easy way for you to stay on top of your blood glucose, blood pressure*, weight, physical activity, and more. The app’s user-friendly interface makes it easy to connect devices, update readings, and see health trends.
