Terms and conditions
360Care is provided by Ellerca Health Corp., 100 Queen Street West, Suite 1009, Brampton, Ontario, L6Z 1A4.
These terms apply to the 360Care app and this website, and to taking part in a 360Care program. Please read them. By using the app or the website, you agree to them.
How we handle your health information is set out separately in ourprivacy policy.
360Care is not for emergencies
If you think you are having a medical emergency, call 911 or go to your nearest emergency department. Do not use the app, and do not message your care team, to report an emergency. Nobody is watching the app around the clock, and messages are answered during working hours.
This includes chest pain, difficulty breathing, signs of a stroke, severe low or high blood sugar, thoughts of harming yourself, and anything else that needs immediate attention.
Who can take part
360Care is for people enrolled in a participating program through their employer or insurer. There is no public sign-up. Your chart is created by your clinic, and you sign in using the phone number or email address your care team already has on file.
You must be 16 or older. The program is not intended for children.
You agree that the information you give us about yourself is accurate, and that you will keep your contact details current so we can reach you.
The care you receive
Your care is delivered by registered dietitians, nurses and health coaches who are licensed in the province where you live. They exercise their own professional judgment, and they are bound by the standards of their regulatory colleges.
360Care supports the care you already receive. It does not replace your family doctor, your specialists, or any treatment they have prescribed. Do not stop or change a prescribed medication because of something you read in the app. Talk to the clinician who prescribed it.
Taking part does not guarantee any particular health outcome. What the program can do depends on your circumstances, and on what you and your care team decide together.
With your permission we will share progress summaries with your family doctor, so everyone is working from the same picture.
What 360Care costs you
Nothing. Where your employer or insurer sponsors the program, the care, the app and the connected devices are included at no cost to you. We do not bill you, and we do not ask you for payment details.
Your access depends on that sponsorship continuing. If your employer or insurer stops offering 360Care, or you stop being eligible under their plan, your access to the program may end. We will tell you before it does, and we will tell you what happens to your health record.
Your account
You sign in with a one-time code sent to your phone or email. There is no password. That code is how we know it is you, so please do not share it, and do not let anyone else use your account.
Tell us at privacy@360care.ca if you think someone else has had access to your account, or if the phone number or email address on your record is no longer yours.
Your devices
If your program includes a blood pressure monitor and a connected scale, they are shipped to your home and they are yours to keep. There is nothing to return if you leave the program.
Please use them as the manufacturer instructs. Any warranty on a device is the manufacturer's, not ours, and we pass on whatever warranty came with it. If a device arrives damaged or stops working, contact us and we will help you sort it out.
Readings from these devices help your care team see how you are doing over time. They are not a diagnosis. If a reading worries you, contact your care team, or seek urgent care if it is serious.
Video visits
Video visits are recorded, and we ask your consent first. If you do not consent, the visit is not recorded. Recording is explained in full in ourprivacy policy.
Using the app and this website properly
You agree not to:
- sign in as anyone other than yourself, or use someone else's code;
- try to access accounts, records or parts of the service that are not yours;
- interfere with the service, probe it for weaknesses, or attempt to get around its security;
- copy, scrape or bulk-collect content or data from the app or the website;
- use the service for anything unlawful, or to harass anyone.
Our care teams are people. We ask that you treat them with respect, and we may end someone's participation if they do not.
If you believe you have found a security vulnerability, please tell us atprivacy@360care.ca rather than disclosing it publicly, and give us a reasonable chance to fix it.
Our content
The guides, articles and other material we publish are ours, or used with permission. You are welcome to read, download and print them for your own personal use. Please do not republish or sell them, or present them as your own.
Anything you write in the app, such as a note about what to focus on in a visit, stays yours. You give us permission to use it to run the service and to provide your care, and nothing more.
Availability
We work to keep the service running, but we cannot promise it will always be available or uninterrupted. We may need to suspend it for maintenance, and we may change or withdraw features as the program develops.

Ending your participation
You can stop taking part at any time. Tell your care team, or ask us to delete your account from Settings > Delete account in the app, or atsymphony.revella.health/account-deletion.
Deleting your account is not instant, and it does not necessarily delete everything. Your clinic may be required by law to keep parts of your health record. A person reviews your request and contacts you to explain what can be deleted and what cannot. This is set out in theprivacy policy.
We may end or suspend your participation if your sponsorship ends, if you use the service in the ways described above under proper use, or where we believe it is necessary for someone's safety. Except where that is not possible, we will tell you why.
Our responsibility to you
Nothing in these terms limits our responsibility for the clinical care you receive, for the professional conduct of our clinicians, for death or personal injury caused by negligence, or for fraud. It also does not take away any rights you have under Ontario consumer protection law or other law that cannot be waived.
What we do limit is narrower. The app and this website are provided as they are. We are not responsible for problems outside our reasonable control, such as an internet or device failure at your end, or an interruption to a service we depend on. We are not responsible for the content of websites we link to.
Where we are responsible for something, our responsibility is limited to what a court would consider the direct and foreseeable loss you suffered.
Changes to these terms
If we change these terms we will update the date at the top and, where the change is significant, tell you in the app before it takes effect.
Governing law
These terms are governed by the laws of Ontario and the federal laws of Canada that apply there. Disputes will be dealt with by the courts of Ontario. If you live in another province, this does not take away the protection of any law of your own province that applies to you regardless.
