What is 360Care™ all about?
360Care™ provides health coaching and support that enables individuals living with diabetes to effectively manage their health. Using 360Care™ conveniently through a mobile device, you’ll have access to a team of health care providers (nurses, dietitians, diabetes educators, psychologists/social workers) dedicated to supporting you through a personalized health plan.
Our innovative solution offers intuitive health coaching and guidance, allowing you to make sustainable, healthy choices that can make a big difference in your life.
How does 360Care™ work?
We understand that living with diabetes isn’t easy. People with diabetes are often told to fix their diet, exercise more, watch their weight, check their blood sugars and more. On top of everything else that you need to accomplish, these little tasks can become big obstacles, especially if little guidance or resources have been provided.
360Care™ provides you with ready-access to a dedicated care team of healthcare professionals through your mobile device, excited to work with you, to support, encourage and inspire you in a manner that best works for you. Our care program includes the following elements meant to maximize our ability to help you to change your behaviours and learn the skills you need to help better manage your diabetes.
How can 360Care™ improve my life?
Live coaching services with regulated health professionals are at the core of our offering. You’ll receive a holistic perspective on the status of your health through coaching sessions with our Heath Navigators (nurses, RN/CDE), dieticians, and psychologists/social workers). Our coaches will work with you to make small, incremental changes that last.
Key areas of attention are;
It’s common to feel overwhelmed, sad, or angry when living with diabetes. You may know the steps you should take to stay healthy, but have trouble sticking with your plan over time. Our coaching teams will work with you to learn to cope with your diabetes and make the types of changes that will allow you to live a healthier, happier life.
How much does 360Care™ cost?
360Care™ partners with several employer health, benefits and EAP plans to provide health coaching to Canadians. Contact your benefits epartment or call the member services number on the back of your insurance card to see if you qualify for 360Care.
Is 360Care™ available to my family member?
Absolutely — If your family member is covered under your benefit plan.
What are the qualifications of your health coaches?
Our nurses are certified diabetes educators with expertise in counselling and managing individuals with diabetes and other chronic conditions such as high blood pressure, cholesterol, heart disease and obesity.
Our care teams also include registered dietitians, who assist clients by working with them to identify their dietary constraints and barriers, followed by the development of diet plans or diet modifications.
Our psychologists and social workers possess clinical skills and professional training that enable them to distinguish between a mental health diagnosis and chronic disease symptoms which are often mistaken for one another.
By using evidence-based treatment protocols, our clinical team help will help you to understand your thoughts, emotional experiences and behaviours to find healthy solutions to problems.
Which languages is 360Care™ available in?
Currently, our Concierge and 360Care™ coaching services are available in both English and French.
What is your cancellation policy?
If you no longer want to continue, you can easily cancel at any time through your account settings. Once you cancel, your account will be marked to not renew. If you cancel before your plan expires, you will still have access to the platform for the remaining time you’ve already paid for.
Is 360Care™ covered by my workplace benefits?
Yes. 360Care™ also partners with several health plans to provide online therapy to employees through employee assistance programs (EAP) and extended health benefits. Contact your benefits department or call the member services on the back of your insurance card to see whether you qualify for this benefit.
Once I sign -up, do I keep the same health coach throughout the program?
During your initial consultation, you will be asked a few simple questions so the agent can gather some information about you and — based on their training and experience — use that information to match you with coaching team members who they feel is best able to help you.
We are careful to find the best match since the experience is different than simply picking someone out of a directory. We know the client-coach relationship is very important, so this matching process is a service we are proud to provide at 360Care.
